Please ensure that you have taken the following actions:
- Make sure the information you seek is not already public. We suggest that you search the U.S. Department of State’s website, OIG's reports, and OIG's FOIA Reading Room before submitting a request.
- Make sure you are looking for DOS/OIG records. Each federal agency has an OIG and this office only maintains records pertaining to OIG's mission of conducting independent audits, inspections, evaluations, and investigations of the Department of State and the U.S. Agency for Global Media. FOIA contacts for each agency are available at foia.gov.
- Information about submitting complaints to the OIG Hotline can be found here.

Requesting Your Own Records
To request records about yourself, you are required to submit with your request a certification of identity. OIG will not process requests from individuals seeking information about themselves if this requirement is not met. This helps OIG ensure that private information about any individual is not improperly disclosed to third parties. For your convenience a certification of identity form is available for you to complete and provide with your request.
Requesting Someone Else’s Records
To request records about another individual, or on behalf of another indivual, you are required to submit with your request certification of identity completed by the individual whose records you seek, and a statement from that individual allowing us to release their records to you. If you do not or are unable to provide authorization, your request will be processed; however, please note that we typically cannot confirm or deny the existence of third party records withour proper documentation. For your convenience a certification of identity form is available for you to have completed and provide with your request.
